Camp YI must also have a current health history form on file.  If you completed one in 2025, you can bypass this step below. If you have not, you must complete a full registration with health history form by clicking on the link provided below.

IF YOU ARE A NEW CAMPER OR BRINGING A FRIEND, YOU AND/OR ANY NEW CAMPER WILL NEED TO COMPLETE A SEPARATE CAMPER REGISTRATION WHICH INCLUDES A HEALTH HISTORY FORM.  CLICK HERE TO COMPLETE.
